A 78-year-old suffered a work injury. Because the worker was a quadriplegic, they needed wound care for a pressure ulcer on the right plantar heel. Until Paradigm started working on the file, the patient was being transported three times a week to the wound care clinic, and weekly to their physician appointments.
Through our network of credentialed providers, we identified a specialized wound care Advanced Registered Nurse Practitioner (ARNP) and brought that person into the home. Every subsequent visit consisted of the ARNP taking care of the wound. During each visit, the ARNP would also measure, photograph, and document the heel ulcer injury. Our team kept all stakeholders involved in the case informed every step of the way.
After reviewing the worker’s medical documentation, physician orders, and home health treatment plans, the case manager determined that the injured worker could receive biweekly wound care treatments at home. Performing the necessary care at home eliminated the need to transport the worker to the wound care clinic. The physician appointments were also no longer necessary, since the ARNP was able to write her own orders.
The worker’s wound was completely healed six weeks ahead of the authorization date—and the worker was very pleased that their wound care was able to take place at home, rather than at the wound care center. For this referral, Paradigm saved the client an estimated $16,370.
