This year, May 9-15 is National Skilled Nursing Care Week (NSNCW). Started by the American Health Care Association, NSNCW highlights the role of skilled nursing facilities (SNFs) in caring for our nation’s injured workers and others requiring care. The theme for 2021, “Together Through the Seasons,” honors SNFs’ collective commitment to deliver compassionate care every day.
Paradigm is proud to recognize the passion and dedication of those who work at these valuable facilities. With the overwhelming challenges of the pandemic, these dedicated front-line workers have shown an extraordinary commitment to quality care while helping injured workers restore function and return to work.
Paradigm would like to acknowledge and thank the SNF providers in our Post-Acute Care Network for their contributions in providing exceptional care for injured workers. Skilled nursing care is essential for helping those patients dealing with a wide range of work-related injuries achieve positive outcomes. From post-surgery physical rehabilitation to ventilator management, it takes highly specialized skills to help injured workers return to their lives and communities.
Recognizing the impact of skilled nursing facilities on post-acute care
Our growing network comprises over 2,000 SNF providers and more than 15% of those facilities have been awarded the prestigious American Health Care Association and National Council on Assisted Living’s National Quality Award. This honor, which is bestowed upon only 6% of the nation’s total SNFs, recognizes an unparalleled dedication to patient-centered care and positive outcomes.
Having the highest quality network in place is fundamental to achieving positive outcomes for challenging work-related injuries. But, it has become even more critical in the wake of COVID-19. In responding to the pandemic and its seemingly endless challenges, it has taken a true team effort to continuously ensure that the injured workers we serve receive the care they need. Among our many success stories is a referral managed by Paradigm’s Post-Acute Care Network that involved an injured worker dealing with long COVID-19 symptoms more than 10 months after initial infection. This patient required a highly complex SNF plan of care, including respiratory therapy support, wound care treatment, renal dialysis, tube nutrition, and multidisciplinary therapy.
Paradigm’s Post-Acute Care coordination team reviewed the injured worker’s treatment history and engaged our network provider to achieve a care plan based on comparisons to national functional, behavioral, and cost outcomes. This is just one example that demonstrates how, by working together toward accountable, evidence-based solutions, Paradigm and our SNF partners consistently deliver the exceptional outcomes that get injured workers back to their lives.
