Supporting National Long-Term Care Awareness Month with an Exceptional Post-Acute Care Network

November is National Long-Term Care Awareness Month, an observance recognized by Congress and a number of states to raise awareness of the critical role that long-term care plays in the larger health care system. Started in 2001, this year marks the 20th anniversary of this event.

Paradigm would like to recognize a highly specialized provider — long-term acute care hospitals — which deliver highly specialized health care services to patients with the most acute health conditions and need long-term care services. This can include injured workers and others needing intensive respiratory care requiring continuous mechanical ventilation and complex wound care, and those with health states requiring I.V. antibiotics and nutrition support.

With an aging population, it is estimated that 70% of people aged 65 or older will require some form of long-term care assistance during their lifetime. Additionally, injured workers of all ages suffering from a broad range of injury types often find themselves in need of long-term care services.

In workers’ compensation, high-quality post-acute care facilities help injured workers receive the ongoing care they need. Paradigm Specialty Networks is dedicated to ensuring that injured workers with long-term care needs have access to the right facility for their situation through our network, which includes skilled nursing, assisted living, home health, and long-term acute care facilities.

Our post-acute care network consists of more than 6,300 providers, with locations in 49 jurisdictions. Long-term acute care hospitals in Paradigm’s network willingly participate in industry best practice standards introduced by the Centers for Medicare & Medicaid Services as well as rigorous quality assurance (QA) and continuous quality improvement (CQI) programming. Our network providers may also be accredited by the Joint Commission, the Commission on Accreditation of Rehabilitation Facilities, or on the pathway to achieving certification.

Paradigm Care Coordinators match injured workers to the optimal facility, based on the nature and extent of their injuries. Our expertise means they get the right care, at the right level, and at the right time. In one case, the Paradigm team was able to support a worker with various injuries and conditions, including acute-status post-urinary tract cancer. Paradigm took on the case to manage the long-term acute care hospital care plan and other potential post-acute care services.

After an extensive health history and evaluation, Paradigm worked with the long-term acute care hospital to develop the customized care plan, which included extensive infectious disease I.V. antibiotic treatment and respiratory therapy services. Thanks to the exceptionally coordinated efforts of our post-acute care network and long-term acute care hospital, this injured worker was able to receive optimal care.
